Petition for Probation Continued Clearance (pdf) - must be completed if the student is on Probation Continued status. Status must be cleared before adding, dropping, or withdrawing from classes.

Petition for Suspension Clearance (pdf) - must be completed if the student is on Suspension status. Status must be cleared before adding, dropping, or withdrawing from classes.

Students should print out the appropriate worksheet below and complete it prior to your advising appointment. Once you complete it, you should hold on to it and use the same sheet in future semesters. This sheet helps to keep you on track with all your requirements to graduate.
