Please note that the forms and instruction documents on this page are available in either Microsoft Office (.doc or .xls extensions) and Adobe Acrobat (.pdf) files. For best results, SAVE the files to your computer and then open them. The MS Office forms may be completed on your computer and printed to obtain the appropriate signatures for processing. If you do not have MS Office on your computer, you may print the Acrobat version and type in the information. Upon approval, the completed forms should be submitted to the Office of the Controller for processing.
If you are viewing the Excel formatted forms, you will note that some of these forms also contain embedded instructions and information. If there is a red triangle next to any text, placing the cursor on the text will reveal the information. The hidden information will not print when the forms are printed.
