For assistance in scheduling an appointment, call 601.266.4965.

You will see the following chart on the appointment page. Find an available time slot indicated by a small plus sign in the top-right corner of the box.


Clicking on the plus sign will bring you to this form.
Please fill out all the information in the textbox.

Be sure not to delete any of the existing information inside the text box.
Only click the SAVE button once. You will not see your entry immediately.
In order for the appointment to appear in the calendar it must be approved first.
You will receive an email to confirm your appointment.
