Application for Degree

 The following instructions will help students complete the required form for Degree Application.

  1. Download the Degree Application Cover Sheet here: application cover sheet (doc)
  2. Complete the Application Cover Sheet, sign it and take it to your department for required signatures.
  3. Download the "Exit Survey of Graduate Degree Recipients" (click here),Please complete.
  4. Attach all Required Documents: to include Application Cover Sheet, Exit Survey, *Recepit of Payment and Current Plan of Study (if not on file) and bring/send to the Graduate School
  5. *Click appropriate link below to pay fee with debit/credit card, once you have received your email confirmation of payment, please print it out and it MUST be included with other required documents to be sent to your Graduate Degree Auditor in the Graduate School, McCain Library - room 205.

 

Fees
Doctoral Fee includes dissertation binding fee $325
Masters and Specialist Fee (Thesis) includes dissertation binding fee $100
Masters and Specialist Fee (Non -Thesis)   $50
Deferment Graduation Fee graduate students who request their application be moved to the next semester $50
