Online Forms and Surveys

University Communications can set up an online form or survey for a unit that doesn't have an account of their own on the university's Mach Forms system.

This is especially useful if the unit will not routinely post online forms. For one-off surveys, contact Judy Isbell at 228.214.3295 or at judy.isbell@usm.edu. If you'd like to establish your own account on Mach Forms, fill out an iTech work order.
