Project Request Form

Instructions

• Click the Project Request Form link to open the document.
• Save a copy of the form to your computer.
• Enter the information requested on the form.
• E-mail the completed form to ITPlanner@usm.edu. Your project request will go to Jeff May, Interim Director of the IT Planning Office.

Purpose of the Project Request Form

The purpose of the project request form is to aid the IT Planning Office in identifying the funding source for the project and in determining whether or not the proposed project must be approved through the IT Governance Process. The information provided will also be used to assist the IT Planning office in prioritizing the project request and generating a preliminary cost estimate for the project.

The information provided in the project request form is essential to creating a high-level assessment of the human resources, software, hardware, equipment, consulting services, security, and connectivity needs of the project. The assessment will be used to plan resource allocation, to coordinate the efforts of all departments included on the project team, and to establish timelines and due dates for the project
