Request Form for a New Organizational and Departmental Web Account

Applicants for accounts on The University of Southern Mississippi web servers MUST be full-time faculty or staff of the University. If you are requesting an account for a student organization, please contact your faculty/staff advisor. Also, when giving the name of the applicant, we need the name of the person applying for the account.

All accounts to be created must be authorized by the appropriate department head.

Before requesting an account, please read the Web Account policy. By filling out this application, you agree to abide by the University of Southern Mississippi Web Account Policy.

Once your account is created, you will be notified, and you must pick it up at the Help Desk. If you can not come to the Help Desk (ie, you're at Gulf Coast or Stennis), arrangements can be made for it to be faxed to you. Passwords will never be sent through email.

You may request a specific URL in the section below, where you tell us how you plan to use this web account, and we will make every effort to honor your request. Based on the hierarchical naming structure as well as technical considerations we may not be able to honor your request.
