hearing and vision losses

can request services?
Any parent, family member,
or service/care provider of a child, birth through 21
years, with both vision and hearing losses.

If you have a child that is
eligible for assistance through the MS Deaf-Blind Project,
please print and fill out the following forms in the referral
packet and send them back to us along with supporting
documentation. Someone from our office will be in contact
with you regarding your request.
