To order a copy of an unrestricted tape or interview, print the following form, fill it out, and submit it, along with a check in the appropriate total amount, via United States mail to:

Please write the interviewee’s name, the volume number, and the length of the transcript (or number of tapes) on the order form.  Dollar figures include shipping and handling costs.
