Forms Not Available for Download:

  • Permission to Add After the Deadline -- This is a multi-part carbon form that students can obtain in the dean's or registrar's offices. It must be signed by the student, the instructor of the class you wish to add, and the department chair and dean for the department in which the course is offered.
  • Change of Grade -- When a grade has been entered incorrectly, or when a student has previously been given an Incomplete and now has finished the coursework, the Instructor of the Course completes a form to change the student's grade.
