Applications for Sabbatical and Leave Requests are due to the Provost by October 5, 2009.
This is a reminder that all leave requests (e.g., sabbatical leaves and leaves without pay) for the 2010-2011 academic year are due to the Provost Office by Monday, October 5, 2009.
Requests should include the following items:
- A description of what the faculty member proposes to do during his/her leave. This project description should not exceed two (2) pages, double-spaced.
- A current curriculum vita.
- Letter of recommendation from the department chair (or, at the option of academic departments, from the departmental personnel committee) and the approval of the college dean.
- Copies of supporting documentation, such as letters of support from agencies, universities, libraries, or other institutions with whom the faculty member will be working (optional) or other supporting materials appropriate to the discipline (optional).
- If submitting hard copies of the supporting documents listed above, please deliver to the Office of the Provost, Room 201, Lucas Administration Building.

When a faculty member is granted an academic leave of absence, it is the responsibility of the responsible department chair and dean to ensure that the faculty member's classes are assigned to existing faculty. Employment of additional instructors or faculty for this purpose will be approved only upon the demonstration of substantial need and only if approved by the Provost.
