Please read all of the following information about making an appointment. Failure to follow all the directions may cause you to lose your appointment.

For assistance in scheduling an appointment, call 601.266.4965.

You will see the following chart on the appointment page. Find an available time slot indicated by 1) a grey colored box and 2) a small plus sign in the top-right corner of the box.

Please note that we may have two staff members scheduled at the Hattiesburg location during certain times. If you do not see an available time on the online calendar, you are welcome to call the center to ask about availability: 601-266-4965.

Please fill out all the information in the textbox.

Be sure not to delete any of the existing information inside the text box.
Only click the SAVE button once. You will not see your entry immediately.
In order for the appointment to appear in the calendar it must be approved first.
You will receive an email to confirm your appointment.
