Membership Forms

Regular Membership- Semester or Annual

In order to complete a regular membership, you will need to complete membership form. Forms can be filled out at the Payne Center or print out the forms below and submit to the Payne Center:

Once forms are completed you will need to pay your membership fee at the Payne Center Sales Window.  You may also sign up for locker service if you are interested.

 

Faculty/Staff Membership - Payroll Deduction

In order to complete a payroll deduction membership you will need to complete membership form and payroll deduction form. Forms can be filled out at the Payne Center or print out the forms below and submit to the Payne Center:

  • Membership application form (choose Primary or Secondary user)
  • Payroll Deduction Form (available at the Payne Center)

*If you add locker service you will need to pay a $10 deposit at the Payne Center Sales Window in order to receive your locker number and lock combination.

 

Stop Payroll Deduction

In order to stop payroll deduction, please fill out the Stop Payroll Deduction Form at the Payne Center.  

*If you are stopping locker service, you will need to stop by the Payne Center Sales Window to fill out the appropriate paper work to receive your deposit.
