
College of Arts and Letters Exit Survey
If you are an Honors College student, you must also complete and submit, electronically, the Questionnaire Concerning Completion of Honors Curriculum and Requirements. In addition, you must complete, print, and submit the hardcopy of the Graduation Application for Latin Designation to the Honors College for review and approval. These items are available via the links below. The Honors College will submit approved applications for Latin Designation directly to the Registrar’s Office and will inform the student of that approval by e-mail. The Honors College will also notify students, via e-mail, if their Graduation Application for Latin Designation is not approved.

Graduation Deferment (Rollover) Form
This form is to used used when changing your graduation date from term to another. A $50 fee will be assessed.
