Clinic Services

The MIHDP project ran from 2012-2018. During that time, we provided the following clinic services:

  • Integrated behavioral health services that focus on brief interventions to improve coping skills and quality of life, reduction of troubling symptoms, and providing the necessary support to individuals to improve overall health and well-being.
  • Chronic Care Support services are provided to patients with chronic diseases, such as obesity, diabetes, and hypertension.
  • Coordination of care across multiple systems, routine follow-up between PC visits to monitor goal progression and provide supportive services.
  • Social work-led care teams that provide individualized chronic disease management designed to improve overall health and wellness.
  • Psycho-education designed to improve patient self-management of health issues.
  • Individualized mental and behavioral health interventions to primary care patients.
  • Training of medical and behavioral health professionals and interns in the provision of integrated care.
  • Utilization of other professionals to include Registered Dietitians and Certified Therapeutic Recreation Specialists.



Other Program Components

  • Training and workforce development in integrated primary care.
  • Mental Health First Aid for communities and lay persons.
  • Disaster preparedness training.
  • Technical assistance to primary care and behavioral health providers interested in building an integrated model of care.
  • Program evaluation.
