Last day to submit CITI Completion Report to the Degree Auditor in the Graduate School. Students who have not submitted the CITI Completion Report will not be able to register for future classes until the report has been submitted.

TRANSFER CREDIT APPROVAL FORM (completed by the academic department)    
A student who would like to transfer a course from another institution must provide an official transcript to the department and request approval. Change of Status Form (if applicable, moving conditional admit status to regular admit status) if student earned adequate GPA on 9 hours of graduate coursework.

GRADUATE COMMITTEE REQUEST FORM     
The graduate committee should be formed early in the degree program. As soon as the committee is formed, the Graduate Committee Request Form should be submitted to the Graduate School Degree Auditor. The Graduate Committee Request Form must pre-date the proposal/prospectus defense.

Any changes to the committee will require a new Graduate Committee Request Form to be submitted to the Graduate School (prior to the proposal/prospectus defense).

THESIS, DISSERTATION, NURSING CAPSTONE PROPOSAL FORM     

Upon selection of a research topic and approval by the committee, the student should submit the Thesis, Dissertation, Nursing Capstone Proposal Form to the Graduate School Degree Auditor. 

Please do not upload your proposal/prospectus in Aquila.

APPLICATION FOR DEGREE (Must be completed one semester prior to the planned graduation term)     
Student completes the Application for Degree via their SOAR Self-Service account. The signed degree application and payment receipt should be submitted to the Degree Auditor in the Graduate School. Any student entering prior to Fall 2017 will need to submit a copy of their updated Plan of Study.  

Student must also complete the Exit Survey via their SOAR Self Service account. Doctoral students will need to complete the Survey of Earned Doctorates.
 
Please note: A late fee will be assessed if the Application for Degree is submitted after the published deadline as set by the Registrar's Office.

COMMENCEMENT PROGRAM FORM (DOCTORAL STUDENTS ONLY) -  Commencement Form deadlines are subject to change due to University Communication deadlines     
The information contained in the Commencement Program Form will be used to communicate doctoral candidate details to the Office of University Communications for inclusion in the official Commencement Program.

GRADUATION DEFERMENT FORM    

Last day to submit the Graduation Deferment Form to the Degree Auditor (along with the receipt reflecting payment of the deferment fee*) if the student does not complete requirements for graduation as planned. 

 

*The first deferral fee is waived.

Last day to submit the Results of Oral Defense of Thesis, Dissertation, Nursing Capstone Project Form to the Graduate Degree Auditor in the Graduate School (Nursing Capstones use dissertation form).

SIGNED TITLE PAGE     
Following a successful defense, the student should submit one copy of the signed title page (minus the Dean of the Graduate School's signature) to the Documents Specialist.  The signed title page can be emailed to documentspecialist@usm.edu.  If the academic unit requires a bound copy of the document, the student will be required to provide an original, signed title page on 100% white cotton paper to the Documents Specialist in the Graduate School.

TURNITIN ORIGINALITY REPORT     
Last day for the academic department chair and/or major professor to submit the certification of originality of thesis/dissertation/nursing capstone using Turnitin assessment.  Students should verify with the academic unit that the originality report has been submitted via email to the Documents Specialist prior to this deadline.

FIRST SUBMISSION OF INITIAL THESIS, DISSERTATION OR NURSING CAPSTONE PROJECT     
Last day for students to submit the first copy of their thesis, dissertation, or nursing capstone project to the Documents Specialist via Aquila. The document should be polished for grammar, content, and formatted in the appropriate template model. At this time there should not be any changes to content, grammar, style, etc. unless directed by the Documents Specialist to do so.
