Temporary Illness or Injury

Students with a temporary disability, illness or injury may be eligible for formal accommodations with the Office for Disability Accommodations (ODA). As a courtesy of the University, ODA may be able to provide academic assistance with preferential seating, allowing a student to sit or stand as needed, use of a voice recorder, extra time to take tests and/or to complete in-class assignments. HOW TO PETITION FOR A NOTICE OF ABSENCE:

To petition for a Notice of Absence, please complete this form: Notice of Absence Form

This petition will allow for the offices listed below to authenticate your documentation and provide both you and your instructor(s) with a notice if the documentation submitted is authenticated. This notice does not "excuse" the absence, nor does it guarantee that the student will be permitted to make up tests or other missed assignments. The absence notice simply documents that the student's illness or other circumstances indicate that the student is/was unable to participate in class for the designated period of time. At their discretion, the instructor will make the final decision regarding the Notice of Absence.
