  • The Tiered Fidelity Inventory (TFI) (pdf) is a self-assessment process in which the PBIS Team will evaluate the development and implementation of the critical features of School-Wide PBIS. 
  • The TFI measures fidelity of implementation of Tier 1, Tier 2 and Tier 3 systems.
  • The TFI is designed to give your school feedback for future action planning and is used to determine what level of professional learning in PBIS will be recommended to your school team.
  • SWPBIS Tiered Fidelity Inventory Snapshots (pdf) may be used during the implementation of SWPBIS in order to clearly define each critical element and the research that supports use and potential benefits to overall school climate.
  • School Evaluation Tool (SET) (docx)
    • The School-wide Evaluation Tool (SET) is designed to assess and evaluate the critical elements of SWPBIS and determine the progress made from one academic year to the next.  The SET results are used to:
      1.         determine the degree to which critical elements have been implemented
      2.         identify goals for SWPBIS
      3.         evaluate progress toward school-wide behavior support,
      4.         evaluate current SWPBIS procedures
      5.         compare progress of implementation of SWPBIS from year to year

      The SET should be completed:
      • before the implementation of SWPBIS
      • 6-12 weeks after the implementation of SWPBIS
      • Annually after the implementation of SWPBIS

      The SET is also completed by outside evaluators to determine model PBIS schools.

  • Tier II Assessment and Action Plan (pdf)
    • The Tier II Assessment and Action Plan are completed to evaluate current Tier II behavioral interventions.  Results are used to determine the effectiveness of the interventions in meeting the needs of the students and whether the interventions are implemented consistently.
  • Benchmarks Advance Tiers (pdf)
    • The Benchmarks for Advanced Tiers (BAT) is a self-assessment of Tier 2 and Tier 3 behavior support systems within a school.  It should be completed annually by school level Behavior Support or Teacher Support Teams.  Once the BAT is completed, school teams utilize the results to develop action plans for further implementation of these systems.
  • Leader Self Assessment Team Meetings (docx)
    • The Leader Self-Assessment Team Meetings checklist is utilized as an aid in preparing for, conducting and evaluating SWPBIS Leadership Team meetings. 


  • Booster Readiness Checklist (pdf)
    • The Booster Readiness Checklist is completed by the school administrator after initial implementation of SWPBS but prior to booster training sessions.  It is utilized to determine if necessary elements are in place to sustain the implementation of SWPBS.
  • SWIS Readiness Checklist (pdf)
    • The SWIS Readiness checklist can be completed by the school PBIS Leadership Team or school administrator prior to requesting training for SWIS.  This checklist determines whether the school has all the necessary information, resources, systems and supports needed to utilize SWIS effectively and efficiently.
  • Product Book Checklist (docx)
    • The Product Book Checklist is completed by school PBIS Leadership Teams to determine if the necessary sections of their PBIS Product book are in place or needs improvement.  The checklist should be completed annually and prior to a School Evaluation Tool (SET) being conducted.
  • Team Implementation Checklist (docx)
    • The Team Implementation Checklist is completed monthly by the school PBIS Leadership team to monitor implementation of critical elements of SWPBS.
  • District PBIS Readiness Checklist (docx)
    • The District PBIS Readiness Checklist is completed by district level administration prior to the district beginning PBIS training.  Completion of the items on the checklist indicates that the district has committed time, funding, personnel, and other necessary resources to the PBIS process and thereby insures more positive outcomes.

PBIS Blueprints

  • School-wide Positive Behavior Support:Implementers’ Blueprint and Self-Assessment
    • The purpose of the Implementer's Blueprint and Self-Assessment is to provide guidelines for planning, implementation and evaluating school-wide positive behavior support (SWPBS) systems and process.
  • Evaluation Blueprint for School-Wide Positive Behavior Support
    • The purpose of the Evaluation Blueprint is to aid in the evaluation process of SWPBS.  Utilize it as a guide when developing evaluation questions, selecting appropriate evaluation measures, and using the gathered data for decision-making as well as communicating the results with stakeholders. 
  • Blueprint for School-Wide Positive Behavior Support Training and Professional Development (pdf)
    • The purpose of the Blueprint for SWPBS Training and Professional Development is to provide guidelines for professional development necessary to support SWPBS systems and processes.  It is not a curriculum for training but an outline for the development of professional training.
