Online portal for submitting annual documents 

University Standing Committees must use the Machform online portal to submit any of the following:

  1. Annual documents for University Standing Committees.
  2. Website address for committees that post annual documents to their own USM website.
  3. Request for extension on submission of annual documents.

Even if a committee emails its information, the committee must also submit their documents on the Machform online portal. 

 

Guidelines for annual document submissions

These Guidelines for Annual Document Submissions explain the format and submission procedure for the following required documents, which are due June 30:

  1. Annual Report 
  2. Bylaws (Word format)
  3. Membership List (Excel format)

Statement of interest to serve on a University Standing Committee

Use the Statement of Interest form to indicate your interest to serve on a University Standing Committee.
