Open Enrollment is Oct 1st - 31st.
It is the once-a-year opportunity for benefit-eligible employees to make changes to your pre-tax insurance plans.
You must sign up for Medical Reimbursement and Dependent Care Reimbursement each year as these programs do not automatically carry over to the new plan year. All other benefits will remain the same unless a change is made during open enrollment. All changes will be effective January 1, 2025.
This year all changes, with the exception of health and life insurance, will be made online at www.sabcflex.com/enrollment.
Paper applications must be completed for health and life insurance changes. Visit the link below for the required forms.
If you would like to meet with a Human Resources representative for assistance, an appointment is required. Please click here to make an appointment. (Click on Open Enrollments-Benefits) For Gulf Park employees that would like to meet in person, make sure to click on the Gulf Park option.
All changes to insurance plans must be made no later than October 31st as mandated by our insurance carriers. Health insurance changes must be in Human Resources by Thursday, October 31st at 5:00 p.m. No exceptions
**INSTRUCTIONS FOR ONLINE ENROLLMENT:
Login at: www.sabcflex.com/enrollment
Under Online Enrollment enter your social security number (no dashes) as your User Login/Employee ID #.
For your Password enter your SIX-digit date of birth (mmddyy)
Example: January 21, 1976 would be entered as 012176
Step 1. Review your personal information. If no further changes are needed, click on Continue to Step 2.
If your personal Information is incorrect and you need to update your address, please complete this FORMSTACK.
Step 2. Review your current information on the Insurance Premiums page. If correct and you have no changes, continue to Step 3.
If you wish to enroll in the dental and/or vision, click the enroll button next to the product and complete the online application. Once complete, you will return to the Cafeteria Plan enrollment.
If you wish to change/review your dental or vision coverage, click the change/review button. To cancel any supplemental insurance product, click the cancel coverage button and complete the online form.
Step 3. Make your Flexible Spending election. If you are electing to participate in Flexible Spending, please have your direct deposit information available to enter online. Click on Continue to Step 4.
Step 4. Review your Cafeteria Election and Salary Reduction Summary. If incorrect, click the back button to Step 3. If correct, click on Continue to Step 5.
Step 5. Read the plan provisions, click on the box below Acknowledgement Required and sign in the Employee Signature box by entering your SSN with no hyphens (dashes). Click on Continue to the Last Step.
Step 6. At the bottom of the Election and Salary Reduction Agreement Confirmation page you can retain a summary for your records by selecting Send E-mail or Print.
Step 7. Your enrollment is complete. Click on done.
Health Insurance:
The Plan implemented across the board 5% rate increases for all coverage options. Click on the link below for the updated rates and other plan changes.

- Employees must be approved for the Minnesota life insurance. An application and Evidence of Insurability is required.

**Please complete all personal information on the enrollment forms before your appointment with the benefits teams. Regardless if you are enrolling or waiving the insurance policies, you must complete all forms. Failure to complete all personal information prior to your appointment could result in your appointment being rescheduled.
