Important HealthQuotient Assessment Reminder

Human Resources reminds all participants age 18 and older to complete the HealthQuotient health risk assessment after January 1, 2011 to be eligible for the wellness/preventive benefits. The Web site for the HealthQuotient is https://www.webmdhealth.com/Mississippi.
Those who do not have Internet access can call WebMD at 1.866.789.4594 to request a paper copy. The HQ must be completed each year prior to a wellness/preventive visit for participants to be eligible for these benefits.
