When a Violation Is Detected

Instructor chooses violation level and sanction and completes a Maxient Reporting Form

 

Level I Violations

Option 1

Instructor and student resolve the issue to both parties' satisfaction. Both sign Faculty Resolution Form and instructor submits it to the Academic Integrity Office.

Academic Integrity Office saves all documentation to student file and closes Maxient case file. The closed case allows the Academic Integrity Office to identify patterns of behavior if multiple reports for a student across courses and semesters are submitted.

Option II

Absent a Faculty Resolution Form for a reported violation, the Academic Integrity Office will contact the student. Student has 5 days to respond, either accepting sanction or requesting a facilitator.

Level II & III Violations

A facilitation meeting is required for Level II & III Violations.

Academic Integrity Office will contact the student and provide a student acknowledgement form with these options:

  • I accept responsibility for the reported academic integrity violation and the sanction.
  • I accept responsibility for the reported academic integrity violation but not the sanction.
  • I deny involvement in the reported academic integrity violation.

Facilitation Meeting

OPTIONAL FOR LEVEL I VIOLATIONS. REQUIRED FOR LEVELS II & III.

Facilitation meeting is scheduled. At the conclusion of the facilitation meeting, the facilitator (a representative from the Academic Integrity Council) completes the Academic Integrity Facilitation Form. The form includes the following options for the conclusion of the meeting:

  • The student and instructor agree a violation did not occur.
  • The student accepts responsibility for the reported academic integrity violation and the sanction.
  • The student accepts responsibility for the reported academic integrity violation but not the sanction, and plans to submit an appeal.
  • The student denies involvement in the reported academic integrity violation, and plans to submit an appeal.
  • The student failed to appear at scheduled facilitation meeting.

Appeals

If student chooses to appeal sanction, student must email%20AI%20office for next steps. Student will be asked to submit an updated written statement and an Appeal Initiation Form (provided by the Academic Integrity Office upon appeal) to the Academic Integrity Office office within 5 business days. Instructor has 5 business from date of appeal to submit any additional evidence to Academic Integrity Officer.

 

Academic Integrity Council hearing is set by AI Officer. Hearing takes place at agreed upon date and time.

 

Academic Integrity Council decides sanction is not appropriate OR a violation did not occur. AI Officer notifies instructor to re-calculate grade. Academic Integrity Council upholds sanction. If sanction is an XF, and student has no prior XFs, they may enroll in UNV 300: Professionalism and Integrity to have the X removed. Students can only remove one X during their academic career.
